Impact of multivessel versus single-vessel disease on the association between low diastolic blood pressure and mortality after acute myocardial infarction with revascularization

چکیده

Abstract Background Previous studies demonstrated a J-shaped relationship between low diastolic blood pressure (DBP) and adverse clinical outcomes in patients with acute myocardial infarction (AMI) that was sensitive to revascularization. We hypothesized this differs multivessel disease (MVD) those single-vessel due differing degrees of ischemic burden. Methods Among 9,983 AMI from the Korea Acute Myocardial Infarction Registry database who underwent percutaneous coronary intervention were followed up for median duration 3.2 years, average on-treatment DBP calculated at admission, discharge, every scheduled visit divided into <70 mmHg, 70–74 75–79 ≥80 mmHg. The including all-cause death, cardiovascular (CV) non-CV _hospitalization heart failure analyzed using Cox regression models adjusted covariates. Results In MVD, death (hazard ratio [HR]: 1.47; 95% confidence interval [CI]: 1.06–2.04) CV (HR: 1.59; CI: 1.02–2.46) significantly increased showing relationship. However, these findings not significant disease. On sensitivity analysis excluding subjects baseline SBP <120 an risk mmHg remained MVD. Conclusions revascularization persisted which has high These high-risk require cautious treatment. Funding Acknowledgement Type funding sources: None.

Relationship of Total Homocysteine, Cholesterol, Triglyceride in the Serum and Diastolic Blood Pressure of Patients with Myocardial Infarction

Recent studies have documented the importance of lipids and lipoproteins as determinants of serum total homocysteine concentrations in persons with myocardial infarction, and in healthy subjects. Associations between various biological systems and total homocysteine have also been reported. However, some of these associations are poorly understood. The purpose of our study was to measure serum ...

Effect of revascularization strategy in patients with acute myocardial infarction and renal insufficiency with multivessel disease

BACKGROUND/AIMS The aim of this study was to compare the risk of complications and outcome between infarct-related artery (IRA)-only revascularization and multivessel (MV) revascularization in patients with acute myocardial infarction (MI) with renal insufficiency and MV disease. METHODS A total of 1,031 acute MI patients with renal insufficiency and MV disease who were registered in the Kore...

Effet of low-dose Aspirin on mortality of Acute myocardial Infarction

SUMMARY Between Aug. 1982 and March 1984, In a prospective, randomized trial approximately 24 hours after the onset of acute myocardial, infarction (MI), the influence of Low dose aspirin (120-150 mg/day, just during inhospital phase), on inhospital and post- hospital mortality rate was studied in 62 patients, and compared with control group. The two groups were comparable with regard to age,...

Myocardial perfusion and the J curve association between diastolic blood pressure and mortality.

BACKGROUND The J-curve relationship between brachial diastolic blood pressure (DBP) and mortality is believed to be mediated through reduced myocardial perfusion. This study aimed to determine the relationship between DBP and subendocardial perfusion in patients with and without coronary artery disease (CAD) and to examine central hemodynamic variables that may explain the risk associated with ...
